Our Story
Affectionate Transition Agency was founded on December 17, 2020 by Theodosia Williams. Theodosia has been a registered nurse for over ten years. What inspired her to open a home care agency was when her mother was hospitalized. Her mother underwent a left below-the-knee amputation, but upon discharge from the hospital, the social worker could not find a home care agency to take care of her as per the doctor’s order. This necessitated her taking a month off to care for her mother. And from that moment on, Theodosia decided to open her own home health care agency to assist elders and seniors in the comfort of their own homes. She did not want what happened to her mother to happen to someone else. As a caring and compassionate person, Theodosia continues to make sure that every client under her agency’s care receives the support they need to make the transition to home a smooth-sailing process.
Affectionate Transition Agency is a program approved service ageny (PASA) for intellectual disability/Developmental delay (ID/ DD) under the Medicaid waiver program.

Our mission is to respectfully and compassionately meet the needs of our clients and their families by listening, planning, educating, and delivering the highest quality care. We also aim to see our clients live as comfortably and safely on their own, knowing that they are under the supervision of professionals that are experts at what they do.
Our Vision
Our vision is to see our clients live independently and return to the level of comfort living at home before their hospital stay. We understand that readjusting to the home environment after spending time in a facility has its challenges. We are here to ensure that each client is offered the utmost care to make them feel as comfortable as possible.
